Guidelines support the CCTA funnel. U.S. and European guidance has elevated CCTA in appropriate stable chest-pain and coronary-disease pathways, and the 2026 expert consensus describes CT-derived fractional flow reserve as useful when anatomical findings leave uncertainty and the result can change management. A challenger must do more than train a model: it must validate performance, clear a device, transfer images securely, return dependable results, train physicians, negotiate contracts, support claims, and operate at clinical-service standards. Heartflow’s growth history is exceptional. Revenue increased from $87.2 million in 2023 to $125.8 million in 2024 and $176.0 million in 2025. Growth comes from overlapping sources: CCTA adoption, new account installation, utilization ramps, additional analyses per scan, Plaque reimbursement, office and clinic penetration, payer coverage, and contractual pricing. Public evidence does not provide a clean bridge. The distinction matters because new-account maturation can recur, while a reimbursement launch can create a step-up; product cross-sell can lift gross profit per scan, while lower-priced channel mix can depress revenue per case.
Management began 2026 with revenue guidance of $218-$222 million. After Q1 it increased the range to $228-$232 million. Following Q2 it raised guidance again to $246-$250 million, implying 40%-42% annual growth. Plaque guidance moved to $29-$31 million and approximately 1,250 activated accounts. Longer-term catalysts include trial enrollment, useful decision and clinical endpoints, guideline inclusion, and payer action. Operating expense consumed the gross-profit gain. Sales, general and administrative expense increased from $95.1 million in 2023 to $134.3 million in 2025; R&D increased from $35.9 million to $64.9 million. For the bear case to be right:

Bear falsifiers are equally concrete: independently verified share gains; stable product-level prices; strong multi-year account retention; rising Plaque net revenue per activated account after its anniversary; four consecutive quarters of positive post-SBC operating leverage; cash-flow profitability ahead of mid-2028; annual dilution below 2%; and patent or comparative-outcome evidence that prevents rivals from obtaining equivalent payer access.
The monitoring dashboard should contain unique scans and patients if disclosed, product-level cases and net revenue, retained Plaque accounts, channel mix, gross margin, GAAP operating loss, stock compensation as a percentage of revenue, operating cash use, diluted shares, competitor clearances and coverage, DOJ language, patent rulings, and trial milestones. A thesis should change when those measures change, not when management repeats a market-size estimate.
